About

Dr. Jeffrey Schwartz is a senior Aviation Medical Examiner, certified to complete Class 1, 2, and 3 FAA physicals. A Richmond native, he earned his medical degree from the Medical College of Virginia in 1992. Board Certified in Family Medicine, he currently practices at the rural Virginia Commonwealth University Family Medicine center in Central Garage/King William county. He has been an FAA medical examiner since 2006 and completes FAA physicals in King William as well as downtown Richmond. A retired flight surgeon from the Virginia Air National guard, he has approximately 1000 hours in various military aircraft.

FAA Physical

Class 1, 2, or 3

At Your Appointment:
  • You must bring a picture ID.
  • Bring any/all glasses that you need.
  • You will need to provide a urine sample.

If you have been issued a wavier from the FAA:

  • A Condition the AME Can Issue (CACI) is a waiver from the FAA. Please bring this paperwork and any supporting paperwork from your specialist(s) to your appointment.
  • A Special Issuance (SI) is another type of waiver from the FAA. It has time limits and is very specific about necessary documentation which must be forwarded to the FAA Medical Center. Please bring this waiver paperwork as well as any supporting documents from your specialist(s) to the appointment.

If you don’t have this information, your physical may have to be forwarded incomplete.

EKG

Add-On (Class 1 Only)

$25

The EKG is an add-on to the Class 1 physical and cannot be performed on it’s own.

The physical can be performed with or without the EKG, depending on your requirements.

Color Vision Test

Add-On (Class 1, 2 or 3)

$25

Due to changes in FAA requirements for color vision testing, new applicants and those who may be taking a medication which can interfere with color vision will need computerized color vision testing. 

There will be an additional $25.00 charge for each color vision test, this will be payable at the time of service.

These are important notices of changes in Central Virginia Flight Medicine.  By scheduling your physical, you agree to these changes.

  • Payment for physicals will now be collected at the time of scheduling.  Please be advised that these payments are NON-REFUNDABLE.
  •  If more than 24 hours notice is given to change your scheduled physical, your payment can be applied to your rescheduled appointment.
  • If you reschedule your appointment with less than 24 hours notice, cancel, or do not show for your physical, you will be required to pay the charges again prior to scheduling a new appointment.
  • Due to changes by the FAA on record keeping and required paperwork, there will be $25.00 additional fee for any Special Issuance / Waiver that is processed by CVFM.  That will include any AASI or CACI waiver for any condition.
