Physical with EKG
- If you require an EKG with your FAA physical, it will be a $25.00 additional fee charged at the time of service. EKGs are done in the same appointment slot and cannot be scheduled independently of a physical.
Physical with Color Vision Test
- Due to changes in FAA requirements for color vision testing, new applicants and those who may be taking a medication which can interfere with color vision will need computerized color vision testing. There will be an additional $25.00 charge for each color vision test, this will be payable at the time of service.
Prior to your appointment:
- FAA Med Xpress: MedXPress is the computerized initial portion of your exam. Go to MedXPress at https://medxpress.faa.gov/medxpress to begin your physical. You will need to complete this portion and obtain your MedXpress code number prior to your appointment to complete your physical. If you do not have this number at your appointment, you will have to reschedule as we do not have access to retrieve it.
At your appointment:
- You must bring your government-issued picture ID.
- Bring any/all glasses that you need.
- You will need to provide a urine sample.
If you have been issued a wavier from the FAA:
- A Condition the AME Can Issue (CACI) is a waiver from the FAA. Please bring this paperwork and any supporting paperwork from your specialist(s) to your appointment. If you don’t have this information, your physical may have to be forwarded incomplete.
- A Special Issuance (SI) is another type of waiver from the FAA. It has time limits and is very specific about necessary documentation which must be forwarded to the FAA Medical Center. Please bring this waiver paperwork as well as any supporting documents from your specialist(s) to the appointment. If you don’t have this information, your physical may have to be forwarded incomplete.

- Payment for physicals will now be collected at the time of scheduling. Please be advised that these payments are NON-REFUNDABLE.
- If more than 24 hours notice is given to change your scheduled physical, your payment can be applied to your rescheduled appointment.
- If you reschedule your appointment with less than 24 hours notice, cancel, or do not show for your physical, you will be required to pay the charges again prior to scheduling a new appointment.
